Publicity Release

I agree to allow Midlife Magazine LLC and its related companies, and their respective employees, consultants, licensees, legal representatives, successors, assigns, and those acting with their authority and permission (hereinafter referred to collectively as “MIDLIFE MAGAZINE”) to use my name, picture, likeness, video, testimonial, and biographical data in MIDLIFE MAGAZINE’S advertising, promotional, informational, training, and marketing materials which may be publicly disseminated, displayed, and broadcast by MIDLIFE MAGAZINE.

I hereby grant MIDLIFE MAGAZINE a royalty-free, worldwide, perpetual, non-exclusive license to use any picture(s) and video(s) that I may provide, and such picture(s) and video(s) may be used in accordance with the terms herein. I further grant MIDLIFE MAGAZINE permission, irrevocably and in perpetuity, to reproduce, copy, publish, alter, broadcast, and otherwise use my name, picture, likeness, video, testimonial, biographical data, and any material based on or derived therefrom, or to refrain from so doing, in whole or in part, in color or otherwise, whether accurate or distorted in character or form, without restriction as to changes or transformations, in any manner or media or by any means, methods, and technologies now known or hereafter to be known at any time throughout the world in perpetuity, including without limitation, for all advertising, promotion, information, training, marketing, and other lawful purposes of trade. Such media includes media in any form (whether digital, film, videotape, moving images, stills, or any other form), but is not limited to, print or electronic publications, social media websites (Facebook, Twitter, LinkedIn, YouTube, Instagram, Pinterest, Google +), brochures, magazines, billboards, software, videotapes, DVD’s, CD-ROM’s, movies, broadcasts (television, cable, satellite, and Internet), or otherwise (hereinafter referred to collectively as “PUBLICATIONS”). I understand that MIDLIFE MAGAZINE owns exclusive, unconditional, worldwide, and perpetual rights to the PUBLICATIONS, regardless of the form in which they are produced, published, or displayed, and that MIDLIFE MAGAZINE may use such PUBLICATIONS in any type of media, and to use my name, picture, likeness, video, testimonial, and biographical data in connection therewith. I waive any right that I may have to inspect and approve the finished PUBLICATIONS or any portion thereof.

I agree to forever release, discharge, and hold harmless MIDLIFE MAGAZINE from all claims, demands, lawsuits, and causes of action, whether now known or unknown, that I may have now or in the future, for compensation or benefit, libel, defamation, invasion of right to privacy, invasion of right of publicity, infringement of copyright, or violation of any other right arising out of or relating to MIDLIFE MAGAZINE’S production, display, or use of the PUBLICATIONS.

I certify that I am at least 18 years of age and that this release is signed voluntarily, under no duress, and without expectation of compensation in any form now or in the future.
